While Allstate was trying unsuccessfully to diversify, rivals were revolutionizing the auto insurance business, which Allstate still counts on for most of its sales. Auto premiums generated 55% of its $31.4 billion in revenue last year.Geico and Progressive Corp. found a new formula for growth in the relatively mature business. They crafted a lower-cost model combining direct sales over the Internet and telephone with heavy television advertising. Offering lower prices and more convenience, they grew rapidly at the expense of Allstate and other insurers that sell mostly through agents.Allstates second-place share of the auto insurance market fell to 10.4% from 11.3% over the past five years, while Chevy Chase, Md.-based Geico jumped two points to 8.7% and Progressive climbed to 7.9 %( Suite 101, 2013). State Farm Insurance the largest car and home insurer in the U.S. and a mutual company owned by its policyholders consistently rated better for claims handling and service than Allstate. The claims department of Allstate has some of the most critical interaction with the consumers because it is during a time of need. Consumers contact the claims department when they have suffered a loss to establish a claim. The claims adjuster has the responsibility of obtaining claim pertinent information from the consumer to effectively evaluate the loss. At Allstate Insurance claims departments are separated according to customer needs. Large claims offices known as Market Claim Offices, MCO, are strategically located to provide adequate coverage across the nation. A Market Claim Office presides over a few states or an individual state depending on claim volume and consumer coverage. Allstate has specialized offices that handle claims for the entire nation but only if they meet certain criteria. Strategy and Position Description An automated online estimating system needs to be easy to understand and easy to use. The first element to the estimating tool is it needs to be easy to understand as well as aesthetically pleasing. When a customer logs on to his or her internet profile the option to choose an action needs to include selecting an existing claim on the insurance policy. Once the claim has been chosen, there will be a selection for estimating system. When the estimating system is chosen, the customer will be prompted regarding the damages he or she has sustained. For example, a customer was a victim of a recent theft loss where the front door was kicked in. The customer will select the damage option for entry door which will lead to a chain of questions regarding the  door. Options for the door will include material in which the door is made such as wood, metal, or fiberglass and whether or not the door is painted. Next, the customer will indicate the size of the door, if the customer does not know how to measure a door there will be a help icon that will advise the proper ways to measure a door. Style is another important factor in the cost of entry doors, options such as flush, panel, French, or security are the most common doors available. The next area of focus will include the damage sustained, whether the door, frame, or both are damaged will determine the proper repairs. The last area of concern for an initial estimate is whether the hardware is damaged and what type of hardware the customer currently has. When all of the information has been entered the estimating system will automatically construct an estimate for the removal and replacement of damaged property as well as the disposal of any debris. When the estimate is finished a disclaimer will be needed to inform the customer that the estimate is only a preliminary estimate and if any discrepancies arise when a contractor has seen the damage to inform the claims department for a review of the estimate. Since claims are being filed online, the movement on this is a little quicker. First of all, it reaches the company faster. The need to have the customer come in and get the information then in-putting it into the computer and have it looked at by the investigating agent, then processed, prepared for payment (if need be). Instead, you are cutting out the getting information process, and the inputting it into the computer by the agent. Instead you call or go online and put the information yourself and then let it get processed and then get paid. The affect it has on the movement is that it gets through quicker, you get faster response time from the company, and you get as accurate information as possible. The life cycle in a sense lasts shorter. Instead of taking a few weeks (possibly), it could take up to a week or less. So the life cycle in this case is a shorter one, which is good much better for the company overall. The only downside to this is that even though you get pretty accurate information, you then need to process it into insurance terminology so to speak, in which case you have an agent process the information. It’s also a good thing, because the agent is proof reading it so to speak to make sure that the information posted or given is  correct, and is process correctly, and is given to the proper division within the company. It’s sort of a backup plan and a much needed help for the customer who really has little to no knowledge on how insurance claims work. So overall, it is a faster and easier process which is set up to help the customer, and speed up the process to get claims paid quicker. Over time, while it does become easier, eventually other companies will begin to do this as well, and people will switch to whoever is cheaper. Service Packaging Packaging is not needed for our products or services. Instead we are offering our customers an easier way to file insurance claims. Normally a client would have to call in to the hot line number in order to file a claim. Once the operator answers then the client is expected to listen to numerous options and select which one applies to their personal needs at that moment. This process can be time consuming and aggravating depending on how many clients the customer service representatives the operators are assisting. The next step would be to explain the situation to the operator and wait for them to file the claim for the person. The on line website will be a fast working, easy to navigate website. There is nothing more aggravating then trying to complete information on a website that is slow and takes a long time to accept information. With technological advances, being able to file on line may be more convenient for those who do not have time to call the hot line and file claims. Clients will be able to access the website from laptops, cell phones, iPad, or any other media devices that allow access to the internet. Add value because it will be less time consuming for customers, if a follow-up is needed then the information may be easier to retrieve due to the pre-online registration. With every question there will be a detailed explanation of what information is required for that particular question. The question and additional information will be explained thoroughly to customers without them having to wait on hold in the phone for long periods of time just to speak with a live operator. This will add value to the customers in three ways. The online website will save time. Allow customers to have a more hands-on experience with the editing of claim information. Product Life Cycle Over the lifecycle of a product, a companys differentiation and positioning strategy would need to change as the market, competitors, and product change. When we say that a product has a life cycle we assert four things; Products have a limited life, Product sales pass through district stages, each posing different challenges, opportunities and problems to the seller, Profits rise and fall at different stages of the product lifecycle, and Products require different marketing, financial, manufacturing, purchasing, and human resource strategies in each life cycle stages (Product Life Cycle | Stages and Limitations of Product Life Cycle (PLC), 2014, p. 1). Many firms do not earn profits in this stage. There wouldnt be much price skimming because our product is not a new invention and does have competitors. Due to the amount of competitors in the insurance market, competitive pricing is used. Sales rapidly grow during the growth stage, so we use persuasive advertising to gain customers. Competition is stiff at times prices may be reduced to stay a top competitor. Even though competition may be stiff, firms start to earn more profits in this stage. Our sales increase slowly and the highest figures are reached during the maturity stage of the product cycle. Since competition is at its max level, it would be in Allstates best interest to implement promotional pricing. Our profits are also at their highest level. We use competitive advertising to attract and retain our customers (Market Force, 2005-2014). Next we enter the decline stage where sales and profits decrease. During this stage, we must determine if the decline is temporary or permanent by conducting marketing research. Production may be stopped if the decline is considered to be permanent. If the decline is temporary, extension strategies are  implemented. The extension stage comes into play to come back from the decline. Generally, customer just a roundabout amount of what they would have to pay for car insurance based on their specific coverage needs. Allstate has identified this problem and created an innovative solution. First and foremost, Allstate designed a new way for customers to shop for quotes and have them delivered. According to Marketing Edge (2014), quotes would be delivered anonymously (no personal information required), fast and easy, helpful (accurate), contemporary, and intuitive (para. 4). Allstate needed to spread the word on this innovation, and the best way was to implement a campaign. This campaign included banner ads online and other tools designed to focus on online shoppers. Allstate needed to market themselves differently than their competition in order to stand out, excite consumers, and provoke attention. Once it went live on the www.allstate.com site, DRTV, paid search, and online advertising campaigns were implemented in order to drive traffic to it. The DRTV campaign used a unique URL, getallstate.com for tracking purposes. Online banner ads linked directly to the site so visitors could be tracked through initiation and completion of a quote. The Program Objective: Develop and market a category-unique online shopping/quoting tool and introduce it to consumers in a way that cost-effectively invites greater shopping/quoting in the direct channel (para. 7). This creative strategy had continued to allow online shoppers the freedom  and convenience to stay anonymous. Customers do not even have to provide their name to receive a quote (estimate). There is no application to feel out and no surveys to take. All of the other necessary information is gather by simply inputting your zip code. There are helpful tools strategically placed to assist customers, and the web site design is contemporary but not too flashy. According to Marketing Edge (2014), Based on their information, shoppers are presented with an array of estimates in a matrix that allows them to compare different features and levels of coverage that meet their unique needs. And it provides a price for each option. The tool demonstrates throughout the ease with which consumers can obtain the customized price and coverage information they want (para. 10). Professional Selling Effort In order to market the new automated system for making claims, Allstate needs to express to the customers exactly how beneficial it will be to them. The best approach would be to build the value to the customer. They would have to show the customer exactly how it would benefit them to manage their claims online rather than deal with a claims adjuster right from the get go. Several people will find it convenient to be able to fill out the accident forms right from their mobile device, like their smart phone or tablet, while others will not be so prone to using technology as they are not technologically advanced. We will show the customers not to be afraid of the new systems, and that they are a benefit, not a burden. Initial Budget The budget is a financial plan for Allstate that quantifies the resource allocation plan for the years to come. To start, Allstate would have to create a specific amount, generated from how many people are going to be monitoring the online claims, and how many hours those individuals are going to be putting in. A sum of $208,000 will needed for ten individuals needed for the new system, all set at 40 hours per week and being paid $10 per hour. Allstate will have to create a budget for the person or company putting together the mainframe for putting together the portal for online claims, and setting up the website. Allstate will have to keep in mind that it is going to be a process to put this together and a lot of storage is going to be needed for their clientele. Generally, websites and web design  are going to cost Allstate anywhere from $1,000 t $2,500. Allstate will create jobs for this new system, and they will have to advertise or promote within in order to fill those jobs, b ut this will be very minimal cost to Allstate. The company expects to have a gross profit in the first year of $250,000-$500,000 due to the amount of claims that are not going to need claims adjuster to visit sites to assess the damage personally. The targeted output rate is 1,000,000, and with the average of 86% of all applicants that start the claim online actually finish it, that leaves 860,000 people that use the online claims and pricing. Allstate does not expect any losses with the new market plan. Success Plan Measurement There are several ways to measure the success of Allstates marketing plan. At the bottom of each online article, give readers several options. They can comment or ask questions, request that a lawyer call them, request a white paper or a helpful form or  checklist you’ve developed, or they can register for topical events. Remember, your responsiveness to requests is critical in developing relationships (para. 20). Next, reducing that amount spent on proposals and pitches will help to increase rates in other areas. get to know clients rather than just what they think they want. Allstate must stay one step ahead of the competition to succeed. Many say the perception is reality, so it is imperative to know exactly what Allstates clients think about them. This can be done by implementing satisfaction surveys for the clients to ensure all their needs are met. All threats or problems acknowledged or revealed by the client must be handled swiftly. Reflection on Intramuscular Injections Introduction This paper particularly focuses on my experience of learning from reflection on giving intramuscular (IM) injections, using Gibbss (1988) reflective model. I demonstrate how practice anxiety, as a student nurse, can be dealt with through effective mentoring. I chose the seminal theory of Gibbs reflection on practice, as it illustrates six significant stages; description, feelings, evaluation, analysis of the incident, conclusion and an action plan Ghaye and Lillyman (1997). Gibbs cycle is used throughout the process of reflecting on the incident to help me make sense of my practice and understand what l could do differently to enhance good practice. I use my experience from a placement simulation as I could not be on actual placement due to unforeseen circumstances. Reflection is a process through which healthcare practitioners and students can learn from experience and use the knowledge to inform and improve practice Schon, (1983). The ability to reflect on ones actions is particularly imperative in clinical practice and discourse. As Jarvis (1992) asserted, there is no consensus on the definition of reflection as it is a broad concept. Reid (1993, p305.) define reflection as; a course of action reviewing an occurrence of practice to describe, analyse, evaluate and so inform learning about practice Schon (1983) identified two types of reflection which are; reflection in action, which takes place during the event where the practitioner may not be aware that it is happening and reflection on action, which takes place after the event. Jasper (2003) concluded upon the vitality of reflecting on action, as it transforms experience into knowledge which enhances good clinical practice. Description During my first placement simulation, I practised giving intramuscular injection, is the best tolerated form or injection, and the safest way of injecting medication into a patient Shepherd, (2002). Within the first week of my placement simulation, I was offered the opportunity to practise administering an injection on a dummy. However, as a student, I was cautious and anxious, feeling that I was not competent enough. I discussed my concerns with the lecturer who was empathetic and helped formulate a plan to conquer my doubts. The plan involved a step-by-step conversation of the procedure of administering intramuscular medication, preparing the medication on numerous occasions and practising the injection technique on a dummy. Throughout these stages I was given the opportunity to discuss any questions, feelings or concerns that arose. Once confident enough to do so, under the supervision of the lecturer, I administered an IM injection to a dummy. I recorded my intervention in a refl ective journal. After giving the injection, I was given feedback and the opportunity to discuss my feelings which was valuable and of significance. Feelings As a novice, I never really enjoy giving injections, but after my second practise, good compliments from colleagues and lecturer my confidence increased. Most importantly, l kept thinking, if l gave the injection to an unwilling patient, how awful would I feel? I reminded myself, ethically, what is it like to carry out a therapeutic procedure that inflicts pain on another human being? All these challenges came to my mind before giving the injection. I began to feel quite anxious and nervous, wanting to delay the procedure for as long as possible hoping my anxiety would decrease. I believe the build up of nervousness beforehand is common, particularly for beginners like me. I prepared the injection using the necessary mathematical calculations, picked the right syringe and the right injection for the procedure. I then administered with my lecturer observing my technique, confidence and competence. Evaluation My preliminary anxiety about administering the injection was normal but as a student nurse, I ought to learn the technique and procedure of IM injections. This anxious behaviour could be explained in relation to the Joharis Window (Luft 1969). As in Joharis window, before being confronted with having to administer injection, I experienced an unknown area, whereby my fears and anxieties were anonymous to me and to others [my lecture]. Because I lacked self-awareness regarding these anxieties, I was unable to begin addressing my anxieties around administering the injection. This made me continue to avoid this area of practice, and as a result I did not develop this clinical skill before this incident. By the end of the incident, my feelings were known to me and others [my lecturer]. By disclosing my anxieties and fears to my lecturer and receiving feedback, my awareness of these issues increased. My lecturer commented on what l did right, wrong and what l could have done differently. R esultantly, I was able to address my feelings, areas of strengths, weaknesses and begin to develop the skill of administering injections. Analysis Department of Health (2008) identified lecturers as qualified nurses, who facilitate learning and supervise students. Mentoring also involves the assessment of the student in the practice setting NMC (2008). The interaction between mentor/lecturers and students is answer to minimise practice anxiety and professional intervention is often required to reduce any anxieties. This was resonant in my experience with my mentor while carrying out this procedure. Critically reflecting and understanding my feelings around giving the IM and after the procedure makes me think and evaluate what l ought to do differently in the future. This will enhance my technique and practice while also benefiting patient care. I am now aware of my competence as my mentor highly commended on my confidence, technique quality and the communication that l maintained with her throughout the procedure. Conclusion The description given in this paper is a general and subjective one that attempts to address my feeling and experience of giving the IM. It also aims at indentifying patterns and schemes in the experience of individuals in a similar context of giving IM for the first time. I believe, my experience will help educators to better understand the meaning of the anxiety that is observed as students undertake the act of giving their first injections. It will also help other students reflect and come to understand the meaning of that experience and not feel overwhelmed by this crucial initial experience. Action Plan I created an action plan to improve my practice and set out a course of action should a similar incident occur? Recording an entry in a reflective journal enabled me to record areas of practice that I want to develop and will allow me to track my progress regarding the administration of IM injections (Jack and Smith 2007).I also developed listening skills. Stickley and Freshwater (2006) argue that effective communication, which involves active listening, is an essential nursing skill, because of its beneficial effects on the patients experience.

Like the other novels I read in the class, the images in the Between the Acts cannot be separated with the story development, and the images themselves construct the story in the book by dismantling the conventional expectation for the novel. This novel constructs the images and the representation with their conventional words and actions of the characters. The acts in the title of the novel are not only the acts in the play, but also the motion which the characters make and expect, and the motion of the natural sounds and t he silence which the people cannot control the interruption from them. I want to look at how Virginia Woolf uses the words from the people, sounds from the things, and the images of clothes and history for her story in her last novel, Between the Acts. Virginia Woolf's words are not just the tools for her writing but the words themselves are constructing and de-constructing a main plot of the novel. And I think to look the gap between the words and the character's representations who is using the words is the one of the ways to read this novel. This report’s focus is on analyzing the factors that affect the supply and demand of the products. There are several variables that can affect the supply and demand of a product, including consumers’ income, prices of substitute products, costs of inputs and so on. The product discussed for this report is Acuvue, I will analysis how the factors affect the supply and demand of Acuvue. Acuvue is Johnson & Johnson’s Vision Care Product. It is one of the most popular eye contact lenses products for nearsighted correction and eye color alteration. Acuvue has a serial of products that depend on different replacement schedule, such as daily disposable, two weeks disposable, colors and so on. All ranges of the lenses of Johnson & Johnson’s are regarded as comfortable, convenient and good eye health product. (Johnson & Johnson, 2008 )1 In 2007, as a leader and pioneer of eye contact lens producer, there was a sales increase by 17. 6% prior to 2006. The annual expense for normal glasses is $200 dollars but the Acuvue daily disposable is about $480. Take China for example, the GDP of 2007 increased 11. 4% (Wiki, 2008)1, people earned more than before. When people’s income increased, those who could not afford to buy the Acuvue in the past can had enough money to afford it. Moreover, as income increased, people tend to replace their contact lenses much more frequently than before, and the demand of Acuvue increased accordingly. Also, nearsighted people may have much higher requirement for their life quality; so, for those who are tired of wearing glasses or who seek convenience by wearing eye contacts may try to change to use eye contact lenses, therefore, demand for the eye contact lenses will increase. Consumer Preferences Consumer preference is a significant factor which affects the demand of the Acuvue. People have different preference for lenses for nearsightedness correction, such as replacement schedule, material (hard/ soft), and safety. Some people prefer Acuvue Daily for its convenience, and it has no daily maintenance like other types of contact; the user can just throw it away after one day. (Bausch & Lomb, 2008) For some people, they prefer soft contact lenses to hard lenses. In addition, many people care about the safety of eye contact lenses, and daily contact lenses are regarded as the safest and healthiest way, so those people may choose the Acuvue Daily, and thus, the demand of the Acuvue increases. On the contrary, people who are afraid of getting keratitis (wiki, 2008)2 by wearing contacts will buy the glass, and then the demand of the Acuvue will decrease. Moreover, if the nearsightedness’ preference is more favorable to Acuvue, which probably means they have more loyalty to the products, so the existing consumers will seldom change to alternatives and more and more people will buy Acuvue. Price of Substitutes The Substitutes of Acuvue eye contact lenses are glasses and surgery. As compared before, wearing glasses is a more economical way to correct nearsightedness, and there will be lower risk of being involved in keratitis(wiki,2008)2 or other eye problems. Moreover, in the recent years, correcting nearsightedness by taking an eye operation became more popular in many countries. The costs for the surgery are high, and the average for getting the LASIK nearsightedness correction surgery is $2105 in 2008. (All about Vision, 2008) If the price of the surgery decreases, there will be more people choosing eye surgery instead of wearing inconvenient glasses or eye contact lenses; so, there will be less demand of Acuvue. Price of Complementary Goods If the complementary goods of Acuvue increase, the price of the Acuvue will increase accordingly. Complementary goods for Acuvue are the solution of the eye contact lenses or drops for eyes. For the past two years, with the rapid economic growth in China, people have more money to purchase eye contact lens, but if the average prices of the solutions increased dramatically, people who could afford to buy the lenses felt burdened to buy the solution. Advertising Promotion Advertising is a way of communication, Johnson & Johnson get more advertising is to try to persuade potential customers and consumer to purchase more Acuvue(wiki, 2008)3. Johnson & Johnson has released a series of advisements and promotions for Acuvue. They provide free trial pairs of eye contact lens for the potential consumers who can just fill a Free Trial Pair Certificate (Vision Care Inc. , 2008) from the Acuvue official website. In addition, when the â€Å"Acuvue 2 colours† entered the mainland China market, they chose an unknown model to perform the advertisement first, and the sales for the Acuvue 2 was unfavourable. However, then the Johnson & Johnson invite Chinese pop singer Angela Zhang for promotion on TV, then, this product became familiar with lots of youth in China, and more and more people known about the Acuvue and bought it. Consumer Expectations Johnson & Johnson is a well known and trusted company by millions of consumers all around the world. It pays much attention to the safety of their products. So consumers are confident with the Acuvue and more and more people buy it. When the AMO Complete Moisture Plus Muti-Purpose Solution (Anapol Schwartz, 2008) was recalled in May, 2007, people were concerned about the eye safety. From that time, people tented to buy less and use less eye contact lens in order to avoid infection. Number of Buyers If there are more and more potential consumers of Acuvue, demand for Acuvue will increase. For recent years, more and more people in the world have access the computer, and they use computers much more frequently and longer than the past. People are more prone to get nearsightedness. From Johnson & Johnson’s research, there are about 3 million new eye contact wearers in the US every year (Vision Care Inc. )2, so the number of potential buyers is increasing for consuming eye contact lenses year by year. In addition, with the development of technology, the material of contact lenses is getting better and safer, so more people will be confident with the eye-contact lenses so more potential buyers will buy the Acuvue, then the demand for buyers will increase. But if there is new technology that can take place of eye contacts lenses, the nearsighted may give up wearing contacts, so the number of potential buyers decrease which indicates the demand of Acuvue will decrease. Variables of Supply Costs of Inputs The world economy was very unstable since last year, and the price of raw materials kept on rising. Fortunately, there is no news about the raw material price fluctuation for eye contact lenses. If the price of storing contact lenses increases, the Acuvue’s producer Johnson & Johnson must find ways to control the production costs. The company may choose to reduce the production volume of the product and wait for producing that good when the price goes down, therefore the supply of Acuvue will decrease. However when the materials for making the eye contact lenses decreases, Johnson & Johnson will have more capacity to produce more eye contact lenses to increase its supply. Number of competitors The more competitors there are, the less the supply. Due to the property of the vision care industry, the requirement for entering this business field is fairly high. For the current market, there are four main producers and other several smaller producers which produce eye contact lenses. Johnson & Johnson is the largest eye contact lens producer in the world, making sales of $2. billion around the world (Johnson & Johnson, 2008) 3. In China, eye contact lens manufactures are increasing in the market, competition became fierce, raw material for making the contact lenses increased, and the market share became dilute, so although sales of Acuvue increase in China very year, the growth is not as large as expected. New Technology As we know Johnson & Johnson Vision Care first introduced the soft disposable eye contact lens for the world (Wiki, 2008), and this revolutionary product helped the Johnson & Johnson opened a new product field and access to expand their market to over 50 countries in the world. If there is new cheaper material for producing Acuvue, Johnson and Johnson will be capable of producing more Acuvue by the same investment as in the past, which means the supply of the Acuvue will increase. However, in the future, with the development in technology, some existing materials for making eye contact lensse may have potential risks for our eyes. When the Johnson & Johnson found that potential risks really do exist, in order to guarantee the product safety, they had to pay more for preventing the risks harmful to contacts wearer, or even reduce the supply of those products. Suppliers Expect that Future Sales Prices In recently years, there is a trend that more and more reports show that wearing daily disposable eye contact lens is much more comfortable, healthier and safer, because daily replacement lenses are healthier than other replacement schedule. With the expectation, the market for the Acuvue Daily disposable has much more potential than other Acuvue disposable lenses, and Johnson & Johnson will decide to produce more Acuvue Daily in according with the potential increase of the demand for Acuvue. Conclusion To sum up, those variables discussed above with the supply and demand of Acuvue finally affect the price of the Acuvue. As we know, the market price is the equivalent of the supply and demand. It is determined by an â€Å"invisible hand’ of both supply and demand. Therefore, it is impossible for those factors discussed before to singly handedly affect the supply and demand, because the market is complex, all those variables together contributed to our market price.

His genius, says his biographer Walter Isaacson, was the ability to marry observation with imagination, to apply that imagination to intellect and its universal nature. He is properly known as Leonardo rather than da Vinci, although that is common form of his name today. Da Vinci means from Vinci and most people of the day who required a last name were given it based on their place of residence. Leonardo was illegitimate, which, according to biographer Isaacson, may well have assisted his skill and education. He was not required to go to formal school, and he passed his youth in experimentation and exploration, keeping careful notes in a series of journals which have survived. Piero was a well-to-do man, descended from at least two generations of important notaries, and he settled in the town of Florence, marrying Albierra, the daughter of another notary, within eight months of Leonardos birth. Leonardo was raised in the da Vinci family home by his grandfather, Antonio, and his wife, as well as Francesco, Pieros youngest brother only 15 years older than Leonardo himself. Florence (1467–1482) In 1464, Albierra died in childbirth- she had no other children, and Piero brought Leonardo to live with him in Florence. There Leonardo was exposed to the architecture and writings of the artists Filippo Brunelleschi (1377–1446) and Leon Battista Alberti (1404–1472); and it was there that his father got him an apprenticeship to the artist and engineer Andrea del Verrocchio. Verrocchios workshop was part art studio and part art shop, and Leonardo was exposed to a rigorous training program, that included painting, sculpture, pottery, and metal working. He learned the beauty of geometry and the mathematical harmony that art can leverage; and it was here that he learned chiarroscuro, and developed the sfumato technique he would become famous for. When his apprenticeship ended in 1472, Leonardo registered in the Florentine painters confraternity, the Compagnia di San Luca. Many of the works he did in Verocchios workshop were often completed by several of the students and/or the teacher, and it is clear that by the end of his tenure, Leonardo had surpassed his master. Verocchios workshop was sponsored by the Duke of Florence, Lorenzo de Medici (1469–1492) also known as Lorenzo the Magnificent. Some of the works painted by Leonardo in his twenties include the Annunciation and the Adoration of the Magi, and the portrait of Ginevra di Benci. Milan (1482–1499) When Leonardo turned 30, he was sent by Lorenzo on a diplomatic mission to bring a lute in the shape of a horses head that he himself had crafted to be given to Ludovico Sforza, the powerful Duke of Milan. With him was Atalante Migliorotti (1466–1532), the first of his long-term companions, who acted as a friend, assistant, secretary, and romantic partner. When Leonardo arrived in Milan, he sent a letter to Ludovico, a letter that was more or less a job application, laying out in detail the type of job he envisioned being useful to the Duke: military and civil engineering. Instead, Leonardo ended up an impresario, producing elaborate pageants for the royal court such as the Masque of the Planets. He designed scenery and costumes and developed fantastic mechanical elements for the plays that would fly, descend or animate for the audience. In this role, he was part court jester: he sang and played the lute, told stories and fables, played pranks. His friends described him as gentle and entertaining, handsome, precise and generous, a valued and beloved companion. The Genius in the Notebook It was also during this period that Leonardo began keeping regular notebooks. Over 7,200 single pages exist today, estimated to be one-quarter of his total output. They are filled with expressions of sheer genius: flights of fancy, precognitive sketches of impossible technologies (scuba gear, flying machines, helicopters) careful, analytical anatomical studies of dissections he performed on humans and animals; and visual puns. In his notebooks and his canvases, he played with shadow and light, perspective, motion, color. His drawings of humans at the time are fascinating: an old warrior with a nutcracker nose and an enormous chin; grotesquely old men and women; and a thin, muscular, curly-haired androgynous figure, the opposite avatar of the old warrior who would provide centuries of delight and speculation for art historians. Of course, he painted while he was in Milan: portraits included several of Ludovicos mistresses, The Lady with the Ermine and La Belle Ferronnià ¨re; and religious works such as Virgin of the Rocks and the astonishing Last Supper. He also made the famous drawing Vitruvian Man, the best of numerous attempts of the day to illustrate what the Roman architect Vitrivius (ca 80–15 BCE) meant when he said the layout of a temple should reflect the proportions of a human body. Leonardo ditched most of Vitriviuss measurements and calculated his own ideal of perfection. In 1489, Leonardo finally earned the job he had wanted in 1482: he received an official court appointment, complete with rooms (albeit not at Ludovicos castle). His first commission was to make an immense sculpture of the Duke of Milans father Francesco sitting on a horse. He made the model of clay and worked for years planning the casting, but never completed the bronze sculpture. In July of 1490, he met the second companion of his life, Gian Giacomo Caprotti  da Oreno, known as Salai (1480–1524). By 1499, the Duke of Milan was running out of money and no longer consistently paying Leonardo, and when Louis XII of France (1462–1515) invaded Milan, Ludovico fled the city. Leonardo stayed in Milan briefly- the French knew him and protected his studio from the mobs- but when he heard rumors that Ludovico was planning to return, he fled home to Florence. Italy and France (1500–1519) When Leonardo returned to Florence, he found the city still shaken from the after-effects of the brief and bloody rule of Savonarola (1452–1498), who in 1497 had led the Bonfire of the Vanities- the priest and his followers collected and burned thousands of objects such as artworks, books, cosmetics, dresses, mirrors, and musical instruments as forms of evil temptations. In 1498, Savonarola was hanged and burned in the public square. Leonardo was a different man when he returned: he dressed like a dandy, spending almost as much on clothing as he did on books. His first patron was the notorious military ruler Cesare Borgia (1475–1507), who conquered Florence in 1502: Borgia gave Leonardo a passport to travel wherever he needed, as his personal engineer and innovator. The job only lasted about eight months, but during that time Leonardo built a bridge supporting a garrison of troops out of a pile of lumber and nothing more. He also perfected the art of maps, drawing villages as they would be seen from the air, accurate, detailed birds-eye views of cities measured with a compass. He also established a friendship with Niccolo Machiavelli (1469–1527), who would base his classic The Prince on Borgia. By 1503, though, Borgia was running amok, requiring mass executions in the towns he occupied. At first, Leonardo seemed oblivious, but when Machiavelli left, so did Leonardo: back to Florence. In Florence, Leonardo and Machiavelli worked on an astonishing project: they planted to divert the Arno river from Pisa to Florence. The project got started, but the engineer changed the specs and it was a spectacular failure. Leonardo and Machiavelli also worked on a way to drain the Piombino Marshes: water, the movement of it, the force of it, was a fascination for Leonardo throughout his life, but the marsh project was also not completed. Michelangelo Artistically, Florence had a huge drawback: Leonardo had acquired a nemesis, Michelangelo. Twenty years younger, but although as gay as Leonardo was, unlike Leonardo, Michelangelo was a pious Christian convulsed by agony over his nature. The two artists communication devolved into a bitter feud. The two men were each commissioned to do battle scenes: hung in separate galleries, the paintings were depictions of frenzied faces, monstrous armor, and mad horses. Isaacson suggests that the upshot of the war of the battle scene was useful to both artists because they were now both luminaries, rather than interchangeable parts. From 1506–1516, Leonardo wandered back and forth between Rome and Milan; another one of his patrons was the Medici Pope Leo X (1475–1521). In 1506, Leonardo adopted Francesco Melzi, the 14-year-old son of a friend and civil engineer, as his heir. Between 1510 and 1511, Leonardo worked with anatomy professor Marcantonio della Torre, whose students dissected humans while Leonardo made 240 meticulous drawings and wrote 13,000 words of description- probably more, those are what have survived. But the professor died of the plague, ending the project before it could be published. And of course he painted: his masterpieces during this period in his life include the Mona Lisa (La Gioconda); The Virgin and Child with St. Anne, and a series of images of Salai as St. John the Baptist and Bacchus. Death In 1516, Francis I of France commissioned Leonardo for another astounding, impossible task: design a town and palace complex for the royal court at Romorantin. Francis, arguably one of the best patrons Leonardo ever had, gave him the Chateau de Cloux (now the Clos Luce). Leonardo was by now an old man, still productive- he made 16 drawings over the next three years, even if the city project was not completed- but he was visibly ill and had likely suffered a stroke. I covered a large portion of the club with three other security personnel. The owner wanted to resolve conflicts as peaceful as possible, so I was instructed to use verbal persuasion to resolve potential conflicts and to use force as the last option. As a result, my communication skills were honed, and I learned how to be direct and show sympathy at the same time.The biggest test of perseverance in my life happened during my last two years in college. My best friend from high school, both my grandmothers, my two aunts and u ncle died within a span of less than 18 months. Just when I thought that my life hit rock bottom, my mother was diagnosed with cancer and my girlfriend for 7 years broke up with me.Overcoming death and adversity at an early age gives a man a whole new perspective and attitude in life. Making sacrifice for the football team shows that I have loyalty, selfless service and commitment. Rejection Change implementation is never an easy task and without special care taken the rate of rejection to change by clinical and administrative employees is high. In order to achieve successful CQI changes the following guidelines and recommendations are presented. a.) Minimize employee rejection through easily implemented and followed CQI procedures. b.) Engage employees in planning to increase acceptance. c.) Ensure lateral linkages within the organization across specialty departments to improve communication (Sollecito and Johnson, 2013). 2. Statute and regulatory changes With the ever-changing landscape in state and federal statutes and regulations surrounding the health care industry, special attention to this risk area must be taken. In fact, Cohen (2009) stated that â€Å"health care is one of the most heavily regulated of all sectors of commerce† (p. 328). Failure to comply with state and federal statutes and regulations can bring about negative financial affects at KP, including but not limited to; fines, loss of accreditation and credentialing, and an increase in malpractice lawsuits, not to mention a decrease in quality of care. a.) Risk management and quality improvement officers stay current and involved in statute and regulation changes. b.) Mandate educational goals for risk management and quality management officers with regard to state and federal regulations. c.) Implement a monthly employee newsletter within which the risk management officer and quality improvement officer outline regulation changes. Include processes that emplo yees should expect to see implemented to maintain compliance. Include a signature page with those editions that include changes to policy and ensure all employees sign and return to the human resources department. d.) Ongoing training for clinical and administrative employees with regards to statute and regulation. The risk management and quality improvement officers will be responsible to work with the organizations education department to implement new workshops as needed. e.) Include these responsibilities in the performance monitoring strategy for the risk management and quality improvement officers. 3. HAIs Health care associated infections are a serious risk in hospitals, as noted by Sydnor and Perl (2011), in their statement â€Å"HAIs are the most common complication seen in hospitalized patients† (para. 20). Improper prevention can lead to increased costs, lengthier hospital stays, and even patient death. Additionally, a Condition of Participation (CoP) (42 CFR 482.42) by CMS mandates hospital infection control programs to adhere to specific requirements. Recommendations are as follows. a.) Implement a house-keeping checklist to ensure proper sanitization of patient rooms. b.) Implement a sanitization checklist for clinical staff that will enforce hand washing before and after patient contact. c.) Develop a committee to review and revise the KP infection prevention and control program. Revisions should focus on compliance with TJC and the Center for Disease Control (CDC) regulations. Quality Outcomes Internal and external Three internal and external factors that influence quality outcomes are organization management of interpersonal relationships between physicians and patients, patient compliance, and continuity of care. Without proper management of interpersonal relationships between physicians and patients, the organization will face degradation in trust and openness. Patients should be involved in all treatment decisions, through proper education on their diagnosis and treatment options. This will bring about patient engagement in this decision making process. While KP cannot force their patients to comply with treatment guidelines, the external influence of patient compliance is crucial to quality outcomes. Programs focused on thorough training and education of patients and family members will improve the rate of compliance, thus improving the probability of positive quality outcomes for patients. Lack of patient compliance will hinder the treatment process and lower the level of quality outcomes standards at KP. Continuity of care is another internal influence that can affect quality outcomes. Regular follow up with patients will also increase patient compliance. Design new regulatory and statute training programs for all clinical and administrative employees. 2. Review and revise the KP infection prevention and control program 3. Revise the performance management system to include CQI measurements and risk management procedures as performance metrics. Long-Term 1. Increase adherence to state and federal regulations and statutes throughout the KP organization. 2. Reduce HAIs by 10% throughout KP hospitals in the northern California region. 3. Improve CQI measurement and risk management policy adherence throughout the northern California region by 20% among clinical employees. Risk and Quality Management Recommendations Risk management policies 1. Quarterly peer review The Health Care Quality Improvement Act (HCQIA) of 1986 â€Å"encourages hospitals, state licensing boards, and professional societies to identify and discipline physicians, dentists, and other health care providers who, after adequate, nondiscriminatory peer review, were found to have engaged in negligent or unprofessional conduct† (Cohen, 2013 p. 333). Through ongoing screening of new and current clinicians, KP will take responsibility for offering their patients that highest quality of care and reducing the risk of employing negligent clinical employees. 2. Zero tolerance adherence policy for all employees with regard to infection control procedures Part of the CMS CoP (42 CFR 482.42) regulation is the â€Å"designation of an infection control officer and development of relevant policies that address the identification and control of infections and communicable diseases. Without full compliance with all CoPs, KP could face the loss of their Medicare provider agreement. 3. Vulnerability analysis chart and emergency plan policy. According to Rawson and Hammond (2009) â€Å"by evaluating vulnerabilities and taking appropriate preventive action, loss can be minimized in an emergency†Ã‚  (p. 506). Health care facilities should include prevention measures in their emergency plans that include the risk of terrorist attacks. While terrorist attack risk cannot be completely mitigated, it is the responsibility of the health care facility to be prepared for such an emergency. Obtain additional information on including risk of terrorist attacks from the National Institute for Occupational Safety and Health (NIOSH). Quality management policies 1. Adherence to evidence-based clinical guidelines While it has been shown that â€Å"clinicians have customarily enjoyed a great deal of autonomy in their practices† (Argawal, 2010, para 3), it is imperative that clinicians follow evidence-based clinical guidelines. If exceptions should be made the details of, the patient diagnosis and variations should be presented for peer review prior to altering treatment plans. 2. Minimum score of 88% on customer satisfaction surveys Clinicians must maintain an average score of no less than 88% on customer satisfaction surveys quarterly. In today’s health care market, measurements of quality include consumer satisfaction. In fact, Bernard and Savitz (2009) state that in todays â€Å"competitive health care environment, consumers want and expect better health care services and hospital systems are concerned about maintaining their overall image† (p. 185). Relationship between risk and quality management In the past, risk management officers and quality improvement managers worked autonomously from one another, in fact they most often reported to different superiors. However, today healthcare organizations are realizing that in order to reach quality of care goals and maintain effective risk management programs these disciplines must work together closely. An example of how risk management efforts and quality improvement efforts complement one another is seen in the reduction of medical errors. The risk management plan must consider ways to reduce medical errors, while the quality improvement plan will offer solid steps toward minimizing medical errors (Sollecito and Johnson, 2013). Conclusion This summary focused on topics such as the purpose of risk and quality management, risk identification and management, current risks, quality outcomes, organizational goals, and the relationship between risk and quality management. While KP is the nation’s largest integrated health care delivery systems and leader in CQI standardization, there is always room for improvements.
